Pathwise Case Management, LLC provides telephonic case management, transition of care support, chronic disease management, and health education/coaching services to individuals and on behalf of health plans, hospitals, and providers.
Our mission is to help patients and members achieve better health outcomes through coordinated, compassionate care.

Our services are not emergency care.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

2. Use of Services

By using our services or communicating with our team, you agree to:

  • Provide accurate, complete, and current information.

  • Use our services only for lawful and intended purposes.

  • Not use our website or services to engage in fraud, harassment, or unauthorized data collection.

  • Acknowledge that Pathwise operates under HIPAA and other healthcare compliance standards, and that communication is conducted securely and professionally

3. Relationship Disclaimer

Pathwise Case Management does not replace or act as your primary healthcare provider.
Our nurses and care managers offer education, coordination, and support in collaboration with your physicians and health plan.
All medical decisions should be made by you and your licensed healthcare provider.

4. Confidentiality and Privacy

Your privacy is important to us. All personal and health information shared with Pathwise is protected under federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

5. Limitation of Liability

Pathwise Case Management, LLC and its staff shall not be held liable for:

  • Medical decisions or outcomes resulting from services not directly provided by Pathwise.

  • Errors or omissions in information provided by health plans, members, or providers.

  • Any incidental, indirect, or consequential damages resulting from service use or delays.

To the fullest extent permitted by law, you agree that Pathwise’s liability shall not exceed the amount paid (if any) for services rendered.

6. Intellectual Property

All content on the Pathwise website—including text, graphics, logos, designs, and educational materials—is the property of Pathwise Case Management, LLC and protected under copyright and intellectual property laws.
You may not copy, reproduce, or distribute our materials without written permission.
